What Are Over-the-Counter Shampoos?

Over-the-counter (OTC) shampoos are products you can purchase without a prescription from a doctor. OTC shampoos have gentle formulas that help minimize irritation and support the scalp barrier for people with eczema.

Gentle, fragrance-free

These shampoos avoid fragrances, dyes and harsh detergents — common triggers for eczema flares. They’re formulated with mild cleansing agents and designed specifically for sensitive skin. When shopping, look for products labeled “fragrance-free” rather than “unscented,” as unscented products may still contain masking fragrances that can cause irritation.

Moisturizing and barrier support

Often formulated with ingredients like colloidal oatmeal, aloe vera or glycerin, these shampoos help hydrate and soothe the scalp while supporting the skin’s natural barrier function. Many of these products are also sulfate-free, which helps reduce dryness that can worsen eczema symptoms. These moisturizing ingredients work to calm irritated skin while gently cleansing the hair and scalp.

Medicated

For people with eczema who also have overlapping scalp conditions like seborrheic dermatitis, healthcare providers may recommend medicated shampoos. These specialized products contain active ingredients to help manage inflammation and flaking. Common active ingredients may include coal tar, salicylic acid, zinc pyrithione, selenium sulfide and ketoconazole. It’s important to use these products only under the guidance of your healthcare provider.


Shampoos for Highly Sensitive or Reactive Skin

These specialized formulas use minimal ingredients and avoid common allergens, making them suitable for people who react to many products. They’re designed with simplicity in mind, focusing on essential cleansing while eliminating potential irritants. These products are often the best choice for people with eczema who have experienced reactions to multiple hair care products.


What to Look For — And What to Avoid

The best choice depends on the type of eczema and individual sensitivities. In general, when shopping for over-the-counter shampoos for eczema, follow these guidelines:

  • Use gentle, fragrance-free, non-soap shampoos and cleansers.
  • Avoid harsh detergents, dyes and other irritants.
  • Consider those with soothing, moisturizing ingredients for ongoing scalp care.
